Thank you for your interest in registering to become a contributor to the Canine Cruciate Registry.

As well as contributing to the evidence-base, your participation will provide you with enhanced monitoring of your patients following surgery and it will provide you with the tools to audit your outcomes and complications to help with continuous Quality Improvement.

Please complete the information below. Once you submit this information, it will be sent to RCVS Knowledge and Amplitude Clinical Outcomes. Amplitude will use this information to create your user account. RCVS Knowledge will use this information to keep you informed about the project.
